Response to Treatment - August 26, 1993

     Although Seth was apparently calm on this date, the 
illegal restraining to his bed continued.  At some point
during the day, the intern noted that he was is in 
restraints.

     The restraints were kept on right into
the early evening.  The Nursing Data Base Flowsheet
that refers to the restraints is timed from 7am until 7:30pm.
Restraints are not referred to on the night
Flowsheet.

     During the day, a prudent clinician would have 
recognized that something was wrong. While Seth's
heart rate was lower when he was asleep, he was
again in Sinus Tachycardia when awake. In the 
afternoon, his pulse was particularly elevated. See
