Doctors Order for August 27, 1993 -
Contents:

     There were no orders given by Seth's physicians
on the day of transfer to from the Allen Pavilion to the
main campus of Columbia Physicians & Surgeons. There
were standing orders for Ativan 2mg q4hours.

     There was no apparent awareness that an EKG  prior
to transfer showed Seth to still be in Tachycardia. He
received only Ativan, 2mg P.O. at 10A.M. (bookmark),
before the transfer and no medication at Columbia 
Presbyterian.

     Seth was sent without orders for monitoring, straight
from the Intensive Care Unit of the Allen Pavilion to an
open ward at the main hospital.
