Doctors Order for August 26, 1993 -
Contents:
     Rather than the gradual discontinuation of medication
called for by the Standard(bookmark), Seth's 
insufficient intravenous Ativan was discontinued
and switched to the oral form.
     During the morning hours the Vital Signs note
that Seth was asleep. This state of sleep produced an
improvement in the Heart Rate and Blood Pressure.
     However, during the afternoon when the record
notes that Seth was awake, the Blood Pressure was
elevated and his Heart Rate was in Tachycardia. At
2P.M., the Heart Rate was 120BPM
     Most seriously, Seth was still restrained to the bed during
the day. These restraints were also witnessed by the
intern during his\her interview.
     Ativan was never administered in the correct amount
needed to produce deep and continuous sedation. This 
failure to treat resulted in persistent Tachycardia, periods
of Hypertension, on-going Delirium and most seriously,
continuing agitation which  required the prolonged (and
cruel) use of restraints.
