Orders for August 23, 1993 - 
Contents:

     There were orders from both Dr. Berkowitz
and Dr. Collins for this day.
     Dr. Collins, as Dr. Sagman the day before,
was aware that Seth was in withdrawal. As was
the case with Dr. Sagman, his treatment
recommendation was inadequate and he also gave
no instructions for monitoring the clinical response.
     The final note of Dr. Noah Berkowitz indicates
that Seth was to be transferred (I was told by phone
by Dr. Sagman this was to a "special unit where he
would be withdrawn").  No transfer was made, and
this order was never canceled.
